Thames Homecare Care Service’s dementia teams receive specialist training to equip them with deep knowledge and understanding of the disease in its various forms and stages. This knowledge is coupled with ongoing skills training to ensure that our clients have access to the cutting-edge techniques which will help them to live safe and active lives. Our dementia support plans are always based on the needs of the individual clients, putting the person, not the disease, first.
We understand that this disease effects not only the person suffering from dementia, but also their families. We also recognise that welcoming someone into your home to care for a vulnerable loved one requires a great deal of trust. That is why we work closely with the families of all clients to develop tailored care plans and provide regular opportunities for feedback from both parties
If you’re recovering from an illness, injury or operation you may need extra support readjusting to life at home, usually lasting around 6 weeks. Thames HCS Ltd’s reablement service will help you settle in, regain your independence and restore your confidence.
We know that coming home after a debilitating turn can mean a loss of physical strength and general confidence, even with the most ‘ordinary’ of daily tasks and activities. That’s why our reablement teams work in conjunction with a client’s doctors, physios and nursing staff to chart the best course of care to regain lost ability and confidence.
At some stage in our lives, many of us will require assistance in recuperating after an accident, illness, surgery or even prolonged hospital stay. At Thames HCS, we feature specially-trained reablement teams who assist clients of all ages recuperate and regain mobility. In the long-term, our reablement programmes ultimately aim to help reduce the need for on-going care, promoting independence and self-reliance which we see as vital.

As part of the reablement process, we work with a client to set milestones to help achieve independence. Through the process, we’ll work to help a client stretch their limits week-by-week as part of agreed goals with the intent to regain the lost skills and confidence. We’ll of course amend those goals are the programmes progresses as the client’s abilities do.
